{"id":14908,"date":"2021-06-08T11:30:00","date_gmt":"2021-06-08T06:00:00","guid":{"rendered":"https:\/\/idronline.org\/?post_type=article&#038;p=14908"},"modified":"2025-04-23T14:46:36","modified_gmt":"2025-04-23T09:16:36","slug":"vaccinating-pregnant-and-lactating-mothers-is-just-the-beginning","status":"publish","type":"article","link":"https:\/\/idronline.org\/kn-in\/article\/health\/vaccinating-pregnant-and-lactating-mothers-is-just-the-beginning\/","title":{"rendered":"More must be done to protect pregnant women from COVID-19"},"content":{"rendered":"<?xml encoding=\"utf-8\" ?><p>Priyali Sur, an independent journalist who is eight months pregnant, recently launched a <a href=\"https:\/\/www.change.org\/p\/i-urge-shri-drhvoffice-to-immediately-prioritize-vaccinations-for-pregnant-women-vaccines-like-pfizer-and-moderna-that-are-known-to-be-safe-for-pregnant-women-should-be-made-available-in-india-pregandcovid?utm_source=share_petition&amp;utm_medium=custom_url&amp;recruited_by_id=9d12c440-791f-11e8-ac62-f39bd72eb2a3\" target=\"_blank\" rel=\"noopener noreferrer\">petition<\/a> to include pregnant women in India&rsquo;s COVID-19 vaccination drive. Her <a href=\"https:\/\/www.npr.org\/sections\/goatsandsoda\/2021\/05\/18\/997469014\/no-vaccines-supplies-or-hospital-beds-pregnant-with-covid-in-india\" target=\"_blank\" rel=\"noopener noreferrer\">interviews<\/a> with pregnant women and lactating mothers highlight their struggles. Tejaswini, an expectant mother from Bangalore, has been unable to go for her antenatal check-up for more than a month due to a lack of transportation facilities, as well as her fear of contracting COVID-19 at the center. She and her husband are unsure about how to manage the situation.<\/p><p>Tejaswini is not the only mother-to-be facing this dilemma. An <a href=\"https:\/\/populationfoundation.in\/wp-content\/uploads\/2020\/07\/Policy-Brief_Impact-of-COVID-19-on-Women.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">analysis<\/a> of the Health Management Information System (HMIS) data by the <a href=\"https:\/\/populationfoundation.in\/\" target=\"_blank\" rel=\"noopener noreferrer\">Population Foundation of India<\/a> revealed that during the national lockdown (April-June 2020), there was a 27 percent drop in pregnant women receiving four or more antenatal check-ups and a 22 percent fall in prenatal services when compared to the same period in 2019. Furthermore, there was a 28 percent decline in institutional deliveries.<\/p><p>It is estimated that in India, there are approximately <a href=\"https:\/\/timesofindia.indiatimes.com\/india\/allow-vaccines-for-pregnant-lactating-women-obstetric-gynae-body\/articleshow\/82328030.cms\" target=\"_blank\" rel=\"noopener noreferrer\">50 million<\/a> pregnant women and lactating mothers each year. Despite this, pregnant women and lactating mothers were initially excluded from the <a href=\"https:\/\/www.mohfw.gov.in\/\" target=\"_blank\" rel=\"noopener noreferrer\">Ministry of Health and Family Welfare<\/a>&lsquo;s COVID-19 vaccine <a href=\"https:\/\/www.mohfw.gov.in\/pdf\/LetterfromAddlSecyMoHFWregContraindicationsandFactsheetforCOVID19vaccines.PDF\" target=\"_blank\" rel=\"noopener noreferrer\">advisory<\/a> due to the lack of data from clinical trials about the effect of the vaccine on them. It was only on May 19th, 2021&mdash;on the recommendation&nbsp;of&nbsp;the National Immunisation Technical Advisory Group (NTAGI)&mdash;that <a href=\"https:\/\/www.pib.gov.in\/PressReleasePage.aspx?PRID=1719925\" target=\"_blank\" rel=\"noopener noreferrer\">lactating mothers<\/a> became eligible for the vaccine. A <a href=\"https:\/\/www.thelancet.com\/journals\/lancet\/article\/PIIS0140-6736(21)00146-X\/fulltext\" target=\"_blank\" rel=\"noopener noreferrer\">study<\/a> published by Lancet found that pregnant women, especially in the second half of pregnancy, were at an increased risk of complications (severe pneumonia, hospitalisations, admission to intensive care unit, and so on) compared to non-pregnant women of the same age.&nbsp;The Centers for Disease Control and Prevention (<a href=\"https:\/\/www.cdc.gov\/coronavirus\/2019-ncov\/need-extra-precautions\/pregnant-people.html\" target=\"_blank\" rel=\"noopener noreferrer\">CDC<\/a>) in the United States and the <a href=\"https:\/\/www.gov.uk\/government\/groups\/joint-committee-on-vaccination-and-immunisation\" target=\"_blank\" rel=\"noopener noreferrer\">Joint Committee on Vaccination and Immunisation <\/a>(<a href=\"https:\/\/www.gov.uk\/government\/publications\/covid-19-vaccination-women-of-childbearing-age-currently-pregnant-planning-a-pregnancy-or-breastfeeding\/covid-19-vaccination-a-guide-for-women-of-childbearing-age-pregnant-planning-a-pregnancy-or-breastfeeding\" target=\"_blank\" rel=\"noopener noreferrer\">JCVI<\/a>) in the United Kingdom have both classified pregnant women as vulnerable groups who face an increased risk of severe illness from COVID-19.<\/p><blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p>The number of maternal deaths in India is also expected to have increased by 18 percent in 2020 as a result of the COVID-19 pandemic response&mdash;the highest in South Asia.<\/p>\n<\/blockquote><p>Another <a href=\"https:\/\/www.unicef.org\/rosa\/media\/13066\/file\/Main%20Report.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">UNICEF study<\/a> that examined the direct and indirect impacts of COVID-19 on maternal and child mortality revealed that the number of stillbirths is predicted to increase across South Asia, with the largest increase (10 percent) expected in India. Similarly, the number of maternal deaths in India is also expected to have increased by 18 percent in 2020 as a result of the COVID-19 pandemic response&mdash;again the highest in South Asia. Further, child mortality is expected to increase in India by 15.4 percent and neo-natal mortality by 14.5 percent.<\/p><p>It is for these reasons, along with the findings from the <a href=\"https:\/\/www.fogsi.org\/wp-content\/uploads\/covid19\/fogsi-statement-on-covid-vaccination-in-pregnancy-and-bf.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">studies conducted by the CDC<\/a> in the US, that the <a href=\"https:\/\/www.fogsi.org\" target=\"_blank\" rel=\"noopener noreferrer\">Federation of Obstetric and Gynaecological Societies of India<\/a> (FOGSI) has recommended that obstetricians, gynaecologists, and women&rsquo;s health care providers should administer the COVID-19 vaccine to pregnant and breastfeeding women, with preparations to manage adverse events. Despite the recommendations of FOGSI and NTAGI, the government <a href=\"https:\/\/www.pib.gov.in\/PressReleasePage.aspx?PRID=1719925\" target=\"_blank\" rel=\"noopener noreferrer\">has opened up<\/a> the vaccination drive only to lactating mothers and not pregnant women. Additionally, no structured mechanism to monitor adverse events for lactating mothers has been put in place.&nbsp;<\/p><h3 class=\"wp-block-heading\"><strong>Procuring mRNA vaccines and speeding up approval process&nbsp;<\/strong><\/h3><p>To ensure timely protection of pregnant and lactating mothers, the Centre should classify them as a vulnerable group, as has been done in <a href=\"https:\/\/www.cdc.gov\/coronavirus\/2019-ncov\/vaccines\/distributing.html#:~:text=CDC%20is%20tracking%20vaccine%20distribution%20and%20administration&amp;text=CDC%20reports%20COVID%2D19%20vaccine,in%20long%2Dterm%20care%20facilities.\" target=\"_blank\" rel=\"noopener noreferrer\">other countries<\/a>. Countries such as the <a href=\"https:\/\/www.cdc.gov\/coronavirus\/2019-ncov\/vaccines\/recommendations\/pregnancy.html\" target=\"_blank\" rel=\"noopener noreferrer\">United States<\/a>, <a href=\"https:\/\/www.gov.uk\/government\/publications\/covid-19-vaccination-women-of-childbearing-age-currently-pregnant-planning-a-pregnancy-or-breastfeeding\/covid-19-vaccination-a-guide-for-women-of-childbearing-age-pregnant-planning-a-pregnancy-or-breastfeeding\" target=\"_blank\" rel=\"noopener noreferrer\">United Kingdom<\/a>, and <a href=\"https:\/\/codeblue.galencentre.org\/2021\/05\/17\/only-pfizer-vaccine-for-pregnant-breastfeeding-mothers-khairy\/\" target=\"_blank\" rel=\"noopener noreferrer\">Malaysia<\/a> have emphasised administering mRNA vaccines such as Pfizer and Moderna for pregnant women and lactating mothers. This is due to the availability of <a href=\"https:\/\/www.cdc.gov\/coronavirus\/2019-ncov\/vaccines\/recommendations\/pregnancy.html\" target=\"_blank\" rel=\"noopener noreferrer\">data<\/a> and information on the post-vaccination effects and efficacy of the mRNA vaccines on pregnant and lactating mothers from across the globe. Furthermore, as these vaccines can <a href=\"https:\/\/www.ema.europa.eu\/en\/news\/more-flexible-storage-conditions-biontechpfizers-covid-19-vaccine\" target=\"_blank\" rel=\"noopener noreferrer\">be refrigerated<\/a> at 2-4 degrees centigrade and stored for up to a month, India should consider procuring these vaccines on a priority basis for this group.<\/p><div class=\"idron-content\" id=\"idron-1475506361\"><a href=\"https:\/\/idronline.org\/donate\/\" target=\"_blank\" aria-label=\"donate banner\"><img src=\"https:\/\/idronline.org\/wp-content\/uploads\/2023\/08\/Donate-banner-1.jpg\" alt=\"donate banner\"  srcset=\"https:\/\/idronline.org\/wp-content\/uploads\/2023\/08\/Donate-banner-1.jpg 1250w, https:\/\/idronline.org\/wp-content\/uploads\/2023\/08\/Donate-banner-1-300x60.jpg 300w, https:\/\/idronline.org\/wp-content\/uploads\/2023\/08\/Donate-banner-1-1024x205.jpg 1024w, https:\/\/idronline.org\/wp-content\/uploads\/2023\/08\/Donate-banner-1-150x30.jpg 150w, https:\/\/idronline.org\/wp-content\/uploads\/2023\/08\/Donate-banner-1-768x154.jpg 768w\" sizes=\"(max-width: 1250px) 100vw, 1250px\" width=\"1250\" height=\"250\"   \/><\/a><\/div><h3 class=\"wp-block-heading\"><strong>Work that needs to be done on the ground<\/strong><\/h3><h3 class=\"wp-block-heading secondlevel\"><strong>1<\/strong>. <strong>Communication<\/strong><\/h3><p>The <a href=\"https:\/\/pib.gov.in\/PressReleasePage.aspx?PRID=1645363\" target=\"_blank\" rel=\"noopener noreferrer\">National Expert Group on Vaccine Administration for COVID-19<\/a> (a task force dedicated to framing vaccination guidelines) or the MoHFW needs to develop standard operating protocols to spread awareness about the benefits and side effects of the vaccine on pregnant women and lactating mothers. This education needs to happen not just among women themselves, but also among their family members. It is imperative to ensure that informed consent is taken before administering the vaccines. For this purpose, Anganwadi centres and ASHA workers should be involved to drive the communication and messaging.<\/p><h3 class=\"wp-block-heading secondlevel\"><strong>&nbsp;2. Operationalisation&nbsp;<\/strong><\/h3><p>Using existing&nbsp;<a href=\"https:\/\/wcd.nic.in\/schemes\/pradhan-mantri-matru-vandana-yojana\" target=\"_blank\" rel=\"noopener noreferrer\">Pradhan Mantri Matru Vandana Yojana<\/a> (PMMVY) data on pregnant women and lactating mothers from poorer socio-economic backgrounds, either relief camps with doctors and\/or public health care centres (PHCs) can be utilised to administer vaccines to these groups at specified periods. Importantly, these PHCs should register any side effects of vaccination on these women and monitor their health.<\/p><p>Additionally, the Co-WIN portal should provide an option to classify or identify as a pregnant woman or lactating mother and they should be prioritised while providing slots. Separate timings or specific slots on certain days of the week can be reserved for these groups to ensure they are not exposed to large gatherings.&nbsp;<\/p><p>The group can be further incentivised by providing them with an additional benefit of INR 2,000, as is done with PMMVY to encourage health-seeking behaviour.<\/p><h3 class=\"wp-block-heading secondlevel\"><strong>3. Post-vaccination&nbsp;<\/strong><\/h3><p>While lactating mothers have been included in the current vaccination drive, there are no guidelines on communication or monitoring of side effects. This needs to change given the lack of data from clinical trials on vaccination of this section of women in India.<\/p><p>The Co-WIN portal must be updated to ensure there is a mechanism to monitor the adverse effects of vaccination on pregnant and lactating mothers. It can be modelled after the <a href=\"https:\/\/www.cdc.gov\/coronavirus\/2019-ncov\/vaccines\/safety\/vsafepregnancyregistry.html\" target=\"_blank\" rel=\"noopener noreferrer\">V-safe registry<\/a> in the US that collects such data.&nbsp;<\/p><blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p>India&rsquo;s investment in the health and nutrition of pregnant women and lactating mothers has been significantly lower than it should be.<\/p>\n<\/blockquote><p>Additionally, a separate dedicated helpline number for pregnant women and lactating mothers to self-report any adverse effects of vaccination should be set up. Along with this, ASHA and Anganwadi workers should make regular calls, either on a day-to-day or weekly basis to monitor the effects of the vaccine on this section of the population. Importantly, the PHCs should register any side effects of vaccination on these women and monitor their health to address any adverse impact immediately.<\/p><p>India&rsquo;s investment in the health and nutrition of pregnant women and lactating mothers has been significantly lower than it should be. Programmes likes PMMVY have been difficult to access and are <a href=\"https:\/\/idronline.org\/extend-maternity-benefits-to-tackle-child-and-maternal-malnutrition\/\" target=\"_blank\" rel=\"noopener noreferrer\">restrictive in nature<\/a>. Additionally, the investment by the government has been gradually shrinking&mdash;the scheme has been restricted to only the first live birth and in FY <a href=\"https:\/\/www.indiabudget.gov.in\/doc\/eb\/sbe100.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">2020-21<\/a>, only 50 percent of the allocated budget was spent. Our hope is that the national task force takes into account the challenges faced by pregnant and lactating mothers. They should not only include them in the vaccination drive, but also set up adequate measures to ensure their safety through robust communication, monitoring processes, and providing incentives to the
