But for the women who walk into Garima Malik&rsquo;s clinic in New Delhi, it is a very different story. Some cry. Others appear angry, irritable or frustrated. Usually, the cause is domestic violence&mdash;pregnancy is a particularly vulnerable time and as an experienced counselor, Malik is trained to spot the signs.<\/p><p class=\"wp-block-paragraph\">&ldquo;They talk about suicide,&rdquo; she says. &ldquo;Then they calm down. We talk about risk management and safety planning and counseling. So somehow, they cope.&rdquo;<\/p><p class=\"wp-block-paragraph\">Malik says many of those who come to the clinic, run by the medical charity Medecins Sans Frontieres (MSF), have experienced physical and emotional violence during pregnancy. Sometimes this is because they are unwilling to be intimate with a partner in the early part of the pregnancy or immediately after the birth. Other times it is because they have given birth to a girl&mdash;seen by some as less desirable.<\/p><div id=\"idron-4026348357\" class=\"idron-article-in-content idron-entity-placement\" style=\"margin-bottom: 15px;\"><a href=\"https:\/\/idranswers.idronline.org\/\" target=\"_blank\" aria-label=\"What is IDR Answers Page Banner\"><img src=\"https:\/\/idronline.org\/wp-content\/uploads\/2024\/09\/What-is-IDR-Answers-Page-Banner-1.png\" alt=\"What is IDR Answers Page Banner\"  srcset=\"https:\/\/idronline.org\/wp-content\/uploads\/2024\/09\/What-is-IDR-Answers-Page-Banner-1.png 1250w, https:\/\/idronline.org\/wp-content\/uploads\/2024\/09\/What-is-IDR-Answers-Page-Banner-1-300x60.png 300w, https:\/\/idronline.org\/wp-content\/uploads\/2024\/09\/What-is-IDR-Answers-Page-Banner-1-1024x205.png 1024w, https:\/\/idronline.org\/wp-content\/uploads\/2024\/09\/What-is-IDR-Answers-Page-Banner-1-150x30.png 150w, https:\/\/idronline.org\/wp-content\/uploads\/2024\/09\/What-is-IDR-Answers-Page-Banner-1-768x154.png 768w\" sizes=\"(max-width: 1250px) 100vw, 1250px\" width=\"1250\" height=\"250\"   \/><\/a><\/div><p class=\"wp-block-paragraph\">&ldquo;This can cause loneliness in women and they feel frustrated and they feel like he [the husband] needed the child, the family needed the child, and I am the one suffering,&rdquo; she says.<\/p><p class=\"wp-block-paragraph\">Malik says they are the lucky ones&mdash;most Indian women who experience suicidal thoughts in the period during or after their pregnancy will not seek or receive any help. Yet suicide is one of the leading causes of death among women of child-bearing age in India. According to one recent study published in The Lancet medical journal, the suicide rate among Indian women and girls is&nbsp;<a href=\"https:\/\/www.thelancet.com\/journals\/lanpsy\/article\/PIIS2215-0366(21)00152-8\/fulltext#:~:text=Hanging%20is%20the%20most%20common,to%20increase%20risk%20of%20suicide.\" target=\"_blank\" rel=\"noopener noreferrer\">twice the global average.<\/a><\/p><p class=\"wp-block-paragraph\">India has made enormous strides since the turn of the century in reducing overall maternal mortality, reducing deaths by more than half. In 2019,&nbsp;<a href=\"https:\/\/www.pib.gov.in\/PressReleaseIframePage.aspx?PRID=1805731\" target=\"_blank\" rel=\"noopener noreferrer\">103<\/a>&nbsp;mothers were dying per 100,000 live births, down from&nbsp;<a href=\"https:\/\/sansad.in\/getFile\/annex\/217\/Au3108.htm?source=pqars\" target=\"_blank\" rel=\"noopener noreferrer\">254<\/a>&nbsp;in 2004. The United Nations has set the goal of reducing maternal mortality globally to&nbsp;<a href=\"https:\/\/www.who.int\/data\/gho\/data\/themes\/topics\/indicator-groups\/indicator-group-details\/GHO\/maternal-mortality\" target=\"_blank\" rel=\"noopener noreferrer\">70<\/a>&nbsp;deaths per 100,000 by 2030.<\/p><p class=\"wp-block-paragraph\">But that success has exposed a phenomenon that had previously gone largely unnoticed in India: high rates of suicides in the perinatal period, defined as during and immediately after pregnancy. A 2016 study of 462 low-income women in early pregnancy in south India found&nbsp;<a href=\"https:\/\/link.springer.com\/article\/10.1007\/s00737-016-0660-2\" target=\"_blank\" rel=\"noopener noreferrer\">7.6% were at risk of suicide<\/a>&nbsp;compared to roughly&nbsp;<a href=\"https:\/\/womensmentalhealth.org\/posts\/suicidality-perinatal-period\/\" target=\"_blank\" rel=\"noopener noreferrer\">0.4% in the United States<\/a>. Health experts say the government has done little to address this problem, and a suicide prevention action plan devised in 2018 has never been implemented.<br><br>India is losing young women &ldquo;in enormous numbers,&rdquo; says&nbsp;<a href=\"https:\/\/cmhlp.org\/team\/dr-lakshmi-vijayakumar\/\" target=\"_blank\" rel=\"noopener noreferrer\">Lakshmi Vijayakumar<\/a>, a psychiatrist and a member of the World Health Organization (WHO)&rsquo;s International Network for Suicide Research and Prevention. &ldquo;And we don&rsquo;t have any effective mechanism or plan or strategy to address this issue.&rdquo;<\/p><p class=\"wp-block-paragraph\">The Indian government did not respond to a request for comment.<\/p><div id=\"idron-1391477004\" class=\"idron-content idron-entity-placement\"><a href=\"https:\/\/idronline.org\/donate\/\" target=\"_blank\" aria-label=\"donate banner\"><img src=\"https:\/\/idronline.org\/wp-content\/uploads\/2023\/08\/Donate-banner-1.jpg\" alt=\"donate banner\"  srcset=\"https:\/\/idronline.org\/wp-content\/uploads\/2023\/08\/Donate-banner-1.jpg 1250w, https:\/\/idronline.org\/wp-content\/uploads\/2023\/08\/Donate-banner-1-300x60.jpg 300w, https:\/\/idronline.org\/wp-content\/uploads\/2023\/08\/Donate-banner-1-1024x205.jpg 1024w, https:\/\/idronline.org\/wp-content\/uploads\/2023\/08\/Donate-banner-1-150x30.jpg 150w, https:\/\/idronline.org\/wp-content\/uploads\/2023\/08\/Donate-banner-1-768x154.jpg 768w\" sizes=\"(max-width: 1250px) 100vw, 1250px\" width=\"1250\" height=\"250\"   \/><\/a><\/div><p class=\"wp-block-paragraph\">Data on this is limited. India compiles national maternal death statistics by extrapolating from&nbsp;<a href=\"https:\/\/censusindia.gov.in\/nada\/index.php\/catalog\/42687#:~:text=The%20Sample%20Registration%20System%20(SRS,national%20and%20sub%2Dnational%20levels.\" target=\"_blank\" rel=\"noopener noreferrer\">a representative sample survey<\/a>, but does not separate the data into causes of death. Instead, social factors such as early marriage, intimate partner violence, pressure to give birth to a son and women&rsquo;s lack of financial autonomy are drivers.<\/p><h3 class=\"wp-block-heading\" id=\"h-reducing-maternal-deaths-a-revealing-success-story\">Reducing maternal deaths&mdash;a revealing success story<\/h3><p class=\"wp-block-paragraph\">When it comes to the physical causes of maternal deaths, India&rsquo;s success has been marked and is due in large part to an increase in&nbsp;<a href=\"https:\/\/pib.gov.in\/newsite\/PrintRelease.aspx?relid=160828\" target=\"_blank\" rel=\"noopener noreferrer\">deliveries<\/a>&nbsp;at free public health facilities rather than at home. These rose from&nbsp;<a href=\"https:\/\/pib.gov.in\/newsite\/PrintRelease.aspx?relid=160828\" target=\"_blank\" rel=\"noopener noreferrer\">31.1% in 2005-06&nbsp;<\/a>to&nbsp;<a href=\"https:\/\/dhsprogram.com\/pubs\/pdf\/FR375\/FR375.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">88.6% in 2019-21<\/a>, according to government figures, driven by awareness campaigns and by providing small financial incentives to both the women and to grassroots health workers.<\/p><p class=\"wp-block-paragraph\">The southern Indian state of Kerala has been among the most successful in reducing maternal deaths. With&nbsp;<a href=\"https:\/\/www.pib.gov.in\/PressReleasePage.aspx?PRID=1697441\" target=\"_blank\" rel=\"noopener noreferrer\">43 per 100,000 live births,<em>&nbsp;<\/em><\/a>it is the safest place in the country to give birth. It is also the only state to have looked into perinatal suicide data, analyzing the 1,076 maternal deaths registered between 2010 and 2020. During that period, mortality dropped from&nbsp;<a href=\"https:\/\/www.niti.gov.in\/state-statistics\" target=\"_blank\" rel=\"noopener noreferrer\">66 to 43<\/a>, but the share of suicides increased from about 2.6% in 2010 to 6.6% five years later, and 18.6% in 2019&ndash;20.<\/p><p class=\"wp-block-paragraph\">That data should be treated with some caution&mdash;Kerala&rsquo;s relatively low rates of maternal mortality meant the sample size was small. But combined with the 2016 study in south India, it indicates a trend, says Soumitra Pathare, psychiatrist and director of the&nbsp;<a href=\"https:\/\/cmhlp.org\/team\/soumitra-pathare\/\" target=\"_blank\" rel=\"noopener noreferrer\">Centre for Mental Health Law and Policy<\/a>.<\/p><p class=\"wp-block-paragraph\">&ldquo;It is important for various reasons&mdash;we now have systematic data to show that suicides are a significant problem in young women, especially young women who are pregnant or have just delivered a child,&rdquo; he says.<\/p><p class=\"wp-block-paragraph\">&ldquo;Maternal mortality has gone down substantially, because that&rsquo;s something that has had an intervention done for it. So what this actually shows is that we&rsquo;ve not done any intervention for suicide prevention.&rdquo;<\/p><p class=\"wp-block-paragraph\">He cautioned that the data capture only some of the problem. For every person who dies by suicide, an estimated four to 20 times more people&nbsp;<a href=\"https:\/\/www.who.int\/news\/item\/09-09-2019-suicide-one-person-dies-every-40-seconds\" target=\"_blank\" rel=\"noopener noreferrer\">attempt it<\/a>.<\/p><p class=\"wp-block-paragraph\">&ldquo;So the number of attempted suicides [in India] is anywhere between 0.6 million to 6 million,&rdquo; said Pathare. &ldquo;We don&rsquo;t even collect data on it.&rdquo;<\/p><h3 class=\"wp-block-heading\" id=\"h-early-intervention-seen-as-key\">Early intervention seen as key<\/h3><p class=\"wp-block-paragraph\">There has been little research into the drivers of perinatal suicide in India, though the Kerala review identified psychiatric illness, young age, unmarried status and domestic violence as risk factors.<\/p><p class=\"wp-block-paragraph\">Nearly one in every three Indian women between the ages of 15-49 who has ever been married has experienced intimate partner violence,&nbsp;<a href=\"http:\/\/rchiips.org\/nfhs\/NFHS-5_FCTS\/India.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">according<\/a>&nbsp;to government figures. Around 3.1% of women in this category said they experienced physical violence during pregnancy. Marital rape is&nbsp;<a href=\"https:\/\/www.article-14.com\/post\/marital-rape-a-conversation-india-refuses-to-have\" target=\"_blank\" rel=\"noopener noreferrer\">not legally recognized<\/a>, although this is&nbsp;<a href=\"https:\/\/scroll.in\/article\/1015495\/explainer-why-is-marital-rape-not-a-crime-in-india-and-can-the-courts-make-it-one\" target=\"_blank\" rel=\"noopener noreferrer\">being challenged<\/a>&nbsp;in the courts.<\/p><p class=\"wp-block-paragraph\">Nayreen Daruwalla, head of a program on the prevention of violence against women and children at the Mumbai-based non-profit&nbsp;<a href=\"https:\/\/snehamumbai.org\/\" target=\"_blank\" rel=\"noopener noreferrer\">SNEHA<\/a>, says suicide during pregnancy often falls into one of two categories.<\/p><p class=\"wp-block-paragraph\">&ldquo;One is pregnant women who are married and in whose cases the family insists on having a boy,&rdquo; she said. &ldquo;Unwed mothers are a huge category of cases especially given the lack of social support and sometimes the lack of support from the partner who might be reluctant to wed on finding out the woman is pregnant.&rdquo;<\/p><blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p class=\"wp-block-paragraph\">Adding a psychiatric component to support their mental health would help prevent many deaths.<\/p>\n<\/blockquote><p class=\"wp-block-paragraph\">Experts say early intervention is key to preventing perinatal suicides, and that India already has the systems in place to do this.<\/p><p class=\"wp-block-paragraph\">Shaji KS, dean of research at the Kerala University of Health Sciences and part of the team that reviewed perinatal suicides in Kerala, cites India&rsquo;s network of&nbsp;<a href=\"https:\/\/nhm.gov.in\/index1.php?lang=1&amp;level=2&amp;sublinkid=822&amp;lid=218\" target=\"_blank\" rel=\"noopener noreferrer\">grassroots health workers<\/a>, through whom every pregnant person in the country can be reached. Adding a psychiatric component to support their mental health would help prevent many deaths, he said.<\/p><p class=\"wp-block-paragraph\">MSF&rsquo;s Malik sees a need for more vocational training to enable Indian women to become financially independent, making it easier for them to escape abusive situations.&nbsp;<a href=\"https:\/\/journals.sagepub.com\/doi\/abs\/10.1177\/144078339202800105\" target=\"_blank\" rel=\"noopener noreferrer\">Research in Australia<\/a>&nbsp;has found this to be effective in reducing suicides. In India, women&rsquo;s labor force participation has steadily declined from around&nbsp;<a href=\"https:\/\/data.worldbank.org\/indicator\/SL.TLF.CACT.FE.ZS?locations=IN\" target=\"_blank\" rel=\"noopener noreferrer\">30.4%<\/a>&nbsp;in 2000 to&nbsp;<a href=\"https:\/\/data.worldbank.org\/indicator\/SL.TLF.CACT.FE.ZS?locations=IN\" target=\"_blank\" rel=\"noopener noreferrer\">19%<\/a>&nbsp;in 2021.<\/p><p class=\"wp-block-paragraph\">&ldquo;When we talk to such patients, when we talk about leaving husbands and leaving such [a] toxic environment and getting out of this kind of relationship, they want to,&rdquo; she said. &ldquo;They cannot because they are not financially independent.&rdquo;<\/p><p class=\"wp-block-paragraph\"><a href=\"https:\/\/www.sciencedirect.com\/science\/article\/abs\/pii\/S0140673612606060\" target=\"_blank\" rel=\"noopener noreferrer\">Studies<\/a>&nbsp;also show restricting access to pesticides, used in many suicides in India, might prevent some of the deaths.<\/p><p class=\"wp-block-paragraph\">Lakshmi, the psychiatrist and WHO advisor, was part of a taskforce set up by the Indian government in 2018 to suggest ways to reduce overall suicides, whose&nbsp; recommendations have not been implemented.<\/p><p class=\"wp-block-paragraph\">Asked about funding for suicide prevention, the government&nbsp;<a href=\"http:\/\/164.100.24.220\/loksabhaquestions\/annex\/178\/AU3758.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">said<\/a>&nbsp;in February that funds had been allocated and announced plans for a national telemedicine program for mental health. But it did not commit to adopting the task force&rsquo;s recommendations.<\/p><p class=\"wp-block-paragraph\">&ldquo;We have submitted the&nbsp;<a href=\"https:\/\/www.thelancet.com\/journals\/lanpsy\/article\/PIIS2215-0366(21)00152-8\/fulltext\" target=\"_blank\" rel=\"noopener noreferrer\">plan<\/a>&rdquo; said Lakshmi. &ldquo;It is still lying there.
